Pianos are machines. And like all machines, if they are used then they will eventually suffer wear. In the case of a piano this wear can be allowed to accumulate for a century or more. Time alone takes its toll. This is where restoration comes into play: if its broken, then fix it. The problems start with the fact that all pianos were not created equal; some are worth putting more money into than others.
